Supports for Community Living (SCL) Waiver Program
Supports for Community Living Branch, Department for Mental Health & Mental Retardation Services
Overview:
The SCL waiver program is a home- and community-based program funded by the Kentucky Department for Medicaid Services and
administered by the DMR.
The SCL waiver program was developed for Kentucky citizens as an alternative to institutionalization. The focus of SCL is
to allow a person to remain in or return to the community. Persons with mental retardation or other developmental
disabilities who meet the requirements for residence in an Intermediate Care Facility for persons with Mental Retardation
(ICF/MR), and who meet other Medicaid requirements, are eligible for this program.
Supports for Community Living offers the supports and services that are based on the individual's goals, choices and
priorities as identified through an individual support plan. Services can include: Adult Day Training, Case Management,
Community Living Supports, Occupational Therapy, Physical Therapy, Psychological Services, Residential Supports, and Supported
Employment, among others.
Eligibility Requirements:
Persons with mental retardation or other developmental disabilities who:
- meet the requirements for residence in an Intermediate Care Facility for persons with mental retardation (ICF/MR);
and
- meet other Medicaid requirements

- Purchasing/Acquisition
- Selection/Design
AT Devices Provided/Covered:
- Mobility, Seating and Positioning
- Vision/Hearing Equipment
- Other: Home/Worksite Modifications
Application Process:
Application to the SCL waiting list requires that an Application for SCL Waiver and ICF/MR Services form (MAP-620) be
completed and submitted to DMR. This form is accessible from: http://www.mhmr.ky.gov/mr/forms.asp

Bridges & Potholes:
Bridges
- Allows the individual to stay in their community in the least restrictive environment.
- Provides a wide array of services including some assistive technology if the technology is not covered under
general Medicaid, dental or vision programs.
Potholes
- Individuals must qualify for Medicaid in order to access this waiver.
- The program has the option to exclude from the program those individuals for whom there is a reasonable expectation
that this waiver will be more costly than the Medicaid institutional services the individual might otherwise receive.
- Approval of these services does not guarantee that the service, will be available or provided, nor does it
guarantee eligibility for Medicaid benefits.
- These services shall not be provided to individuals while they are inpatients of hospitals, nursing facilities or
ICF/MR/DD.
- For individuals to qualify to receive these services under Developmental Disability, the onset must have occurred
before the age of 22.
