Homecare Program

Department for Aging & Independent Living

Overview:

The Homecare Program helps adults who are unable to perform some activities of daily living and are at risk of institutional care to remain in their own homes by providing support services and coordinating the help of family, friends and provider agencies. Assessment and case management, home management and personal care, home delivered meals, chore services, home repair, respite for family caregivers and home health aide service are among the assistance provided.

Eligibility Requirements:

Participants must be 60 years old or older and:

• unable to perform two activities of daily living or three instrumental activities of daily living,
• be at risk of going to an institution or
• be in an institution but able to return to a private home environment if needed services were provided.

Additional Information:

The program is offered statewide, although some areas may have waiting lists and not all services are provided everywhere.

Bridges & Potholes:

Bridges

• This program enables elderly individuals to live in their home and not in a nursing home.
• There are no income restrictions; a sliding fee scale is utilized.

Potholes

• This program is only available to individuals 60 and over.
• Only pays for a limited amount of home modification, and it is up to each individual ADD district to determine the amount to be spent on the home modifications.
