Acquired Brain Injury (ABI) Waiver Program

Overview:
The ABI Medicaid Waiver Program is designed to provide intensive services and supports to adults with acquired brain
injuries as they work to re-enter community life. Services can only be provided in community settings. Please note that
special financial eligibility criteria for Medicaid are applied for this program.
Fourteen services are funded, including: case management, personal care, companion services, respite care, environmental
modifications, behavior programming, counseling and training, structured day program, specialized medical equipment and
supplies, prevocational services, supported employment, occupational therapy, speech and language services, and community
residential services (excluding room and board). It is expected that upon completing the program, people will be transitioned
to other existing community resources.
Eligibility Requirements:
To qualify for services, an individual must:
- have an acquired brain injury;
- be between age of 21 years and 65 years;
- meet nursing facility level of care;
- be expected to benefit from waiver services; and
- be financially eligible for Medicaid services.

Application Process:
Contact the Brain Injury Services Branch to request the ABI Program Application to be placed on the waiting list. When
funding becomes available, an allocation letter will be sent directing you to choose a case manager. The case manager is
responsible for assisting you with the admissions process and the development of a plan of care.

Bridges & Potholes:
Bridges
- Allows the individual to stay in their community in the least restrictive environment.
- Provides a wide array of services to reintegrate an individual with an acquired brain injury back into the
community.
Potholes
- Individuals must qualify for Medicaid in order to access this waiver.
- This waiver is for a limited time only. The individual must obtain continual support from community resources.
- These services shall not be provided to individuals while they are inpatients of hospitals, nursing facilities or
an intermediate care facility for persons with mental retardation.
- Individuals who qualify to receive these services must have an Acquired Brain Injury and be between 21 and 65.
