Survey purpose and description:

Why is this data being collected?

We are asking that this survey be completed by school districts throughout Kentucky in an effort to gather information that will be used to assess the current state of information technology accessibility awareness. It will further provide a snapshot of the current level of activities being conducted to ensure that schools are procuring and creating instructional technology and related resources that are accessible to students with disabilities and can be used with available assistive technologies.

How will this data be published?

The results of this survey will be released in an aggregate fashion only. No part of this survey will be released in such a way as to identify any individual person or school district. All surveys submitted will be considered confidential.

How will my district benefit from participation in this survey?

By taking the time to complete this survey, you are helping to identify your district’s progress toward providing an accessible information technology environment. Also, aggregate results of this survey will be shared with all participants so you will be able to know how your district compares with others across Kentucky. Furthermore, the resulting information will be useful for planning purposes and could be valuable to you as support data for local grant proposal preparations.

Estimated time to complete this survey:

Between 10 and 20 minutes.
